Qualifications

Provide documentation of HIV positive status
Provide documentation of income at or below 300% of the Federal Poverty level. (Click Here for table)
Have no health insurance or other method of payment, or
Received healthcare service not covered by insurance including Medicaid, or
Have co-payments from healthcare insurance for physician visits, laboratory services and/or medications.

General Restrictions

All Services must be HIV-related
Medications co-payments are covered only by the Consortium and not available through ADAP
In patient services cannot be covered
Payment for services requiring preauthorization is not guaranteed
Payments for services cannot be made to the consumer
All services are subject to the availability of funds
Offered services may vary by agency
